NASB - 41 Kerioth has been captured And the strongholds have been seized, So the hearts of the mighty men of Moab in that day Will be like the heart of a woman in labor.

Although childbirth is a common occurrence, as well as the means by which each of us enters the world, for the woman who is giving birth, it can be (even today, but especially at the time that Jeremiah was writing) painful and dangerous. (According to Genesis 3:16, this was one of the consequences of humanity's fall into sin.) As a result, a woman going through it might well experience apprehension or fear.

In the verse cited in the question, Jeremiah is saying that the judgment that God will visit upon Moab will be so severe that the courage (metaphorically represented by the heart) of even the bravest Moabite warriors will fail them, and they will experience this same kind or degree of fear.
