Is this a promise to Israel only in Exodus 23:25: "Worship the LORD your God, and his blessing will be on your food and water. I will take away sickness from among you."?

NIV - 20 See, I am sending an angel ahead of you to guard you along the way and to bring you to the place I have prepared. 21 Pay attention to him and listen to what he says. Do not rebel against him; he will not forgive your rebellion, since my Name is in him.

Yes. That is an example of being in a covenant with God, 'if you do (x), I will do (x)'. The covenant promises are for 'biblical' Israel (not the "Israel" of today).

None today are in a covenant with God. All today are living in God's 'age of grace' (Eph 3:2), where rather God's "grace is sufficient" in our "necessities" (2 Cor 12:9-10). God's "new covenant" with the yet future "Israel" of prophecy will someday take effect for them:

Hebrews 8:8
"For finding fault with them, he saith, Behold, the days come, saith the Lord, when I will make a new covenant with the house of Israel and with the house of Judah:"
